Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice.  This courtesy on your part will make it possible to give your appointment to another patient.

Health Insurance Plans

Our practice accepts most major insurance plans.  Please call our office to see if we accept your plan.  Please confirm with insurance company if a referral is needed from your primary care physician. 

Billing and Payment Policies

Insurance will be billed and the patient is responsible for any deductible, unpaid balance and co-payment.  Patients also have the option of paying office and surgical fees in advance and then receiving their insurance reimbursement check directly from the insurance company.  For your convenience we also accept the following credit cards:
